It is an HP 812C.

I'm only seeing this with some, not all, FBII applications.

It is a Keyspan PCI/USB board. Keyspan acknowledges difficulties with 
MacOS 8.6 that sometimes cause the Chooser to be unable to associate 
with the USB driver but the latest driver from HP seems to mostly 
eliminate this problem.

If it is a generic problem with the PCI board then why do only FBII 
applications exhibit the problem and not all of them at that? Very 
curious!
