I have a paperback copy of the book somewhere, but I can't find it.
I copied the program from the online version made available on 
this site.

I do have a working copy thanks to Robert C. I sent him the copy
offlist. I had typed it out, described what the program is supposed to
do, and he fixed it up. I would be glad to send you a copy, then you
can study it. You will have to email me directly though, since I don't
have your email address.
